You can only activate such offers when you receive sms or calls(IVR or Customer Care) regarding the special schemes. It has been a long time(about 6 months) since I heard someone getting such offers. Anyway just call customer care and ask whether any offers are applicable to you.

Yes, if you recharge with any normal top up amounts you will be charged ₹20 for reactivation and your old package will start on DBR basis.
