HD Access Fee is applicable only on the SD channels which are part of base packs and not for channels from Add-On or A-la-carte.

Here D2H Cinema is A-la-carte and hence HD Access Fee is not applicable and you will not get D2H Cinema HD. You have to buy it separately.
