What is the meaning of this? Is there a penalty if operators don't follow?
Suppose LCN 552 is Gemini, 553 is Star Maa, 554 is Zee Telugu, 555 is ETV. Then ETV is not in the top 2. Is there a penalty?

It all depends upon what agreement done by Both Operator n broadcasters .

in case operator has not allocated lcns due to LCN unavailability or some other issues though,as per their(broadcaster) slab then they should pay more to get paid channels. k:
