Set 4 : 3 in set top box as you have CRT 4:3 TV

And how does set top box come to know that the picture is in 16:9 or 4:3 ratio?

My cable SD STB has option for 4:3 & 16:9, but the option doesn't work with CRT or LCD.
So in CRT it shows normal 4:3 & on LCD it shows horizontally stretched distorted picture.
Cable HD STB also has option for 4:3 & 16:9. It works perfectly on CRT as well as LCD.
So on CRT, it shows 4:3 LB picture for HD channels. Also on LCD, it shows 16:9 PB picture for SD channels.
I think STB comes to know about the picture format from the signal/frequency & accordingly it adjusts the output format.
